In order to use the form you must download it on your
computer, save the changes and e-mail the final copy to

Change of
Major/Minor Form |
This form allows business students to change your major or minor in the School of
Business. You must already be in the School of Business to change
majors. After the form is completed, a faculty advisor will be assigned
to you from the department offering your major. |

Transfer Credit Agreement Form |
This form must be filled out before you take classes at
another college or university. Please allow up to 2 weeks for this form to
be processed. |

Change of
Cluster Form |
This form allows you to declare or change your thematic cluster. For more
information on clusters click

Intent to
Transfer Form |
This form is for students that are currently enrolled at the
University of Dayton who want to transfer to the School of Business.

The only way to get this form is to attend a counseling meeting.
You can schedule a meeting by calling 229-2259. Meetings are held on
Tuesdays at 11am and Wednesdays at 4:30pm. |

Add/Drop Slip |
This form allows students to add or drop a course or change grading
options. Please check registrar.udayton.edu for deadlines. If you need a
signature, stop by the Advising Center, MH 211. |
